There have been significant changes to the new W-4 form. There are no more allowances due to changes in the law. Current employees are not required to submit a new W-4; however, if they choose to make changes, they must use the new form. All employees hired after January 1, 2020 must use the new form.   • This form is needed if you are a current FWSU employee and want to rollover funds to Aspire.

• 403B Separation of Service Packet

  • This form is needed if you are a former FWSU employee and want to rollover funds from Aspire. This form and all required supporting documents can be uploaded through the form submission tool.
